Jaipur, Aug. 4 -- Rajasthan will establish 80 additional specialised Type-1 diabetes clinics over the next three months, expanding its flagship Mission Madhuhari to strengthen care for children and young people living with the chronic condition, the state government announced. A series of technology-driven initiatives, including the country's first "MadhuRoop AI Digital Twin" platform and a 24x7 counselling service through the 104 health helpline will be facilitated, positioning Rajasthan as a pioneer in AI-enabled public healthcare, they said. The initiative aims to ensure that no child dies due to Type-1 diabetes by 2030. According to the Medical and Health Department, Mission Madhuhari has emerged as a comprehensive public health programme integrating treatment, digital monitoring, awareness, mental well-being and artificial intelligence for patients across the state. Rajasthan is estimated to have nearly 53,000 people living with Type-1 diabetes. To address the challenge, the state currently operates 120 dedicated Type-1 diabetes clinics across all 41 districts, with another 80 clinics set to become operational within three months, taking the total network to 200 specialised centres. So far, 2,817 patients have been registered under the programme and are being digitally monitored through the Mission Madhuhari mobile application. The department said 99% of registered patients are following the recommended basal-bolus insulin therapy, while thousands are undergoing regular HbA1c testing and continuous clinical follow-up. Officials said the Centre has also incorporated active screening for Type-1 diabetes in schools into its guidelines based on Rajasthan's experience under the programme. The government now plans to extend the mission to private hospitals to ensure digital registration and comprehensive treatment for every patient. Principal secretary, Medical and Health, Gayatri Rathore, said the programme began with a comprehensive assessment of the state's healthcare infrastructure in March 2024. A pilot project, implemented in collaboration with the William J Clinton Foundation, was launched in June 2024 at selected hospitals in Jaipur, Jodhpur, Udaipur and Nagaur before being expanded across Rajasthan. The initial assessment identified several challenges, including lack of standardised treatment protocols, shortage of trained healthcare personnel, unequal insulin availability and weaknesses in the referral system. These gaps were subsequently addressed through Mission Madhuhari. Under the programme, patients receive free insulin, glucometers, test strips, lancets and logbooks. This year, the government has also arranged Continuous Glucose Monitoring (CGM) devices for nearly 4,000 patients, enabling real-time blood sugar monitoring. More than 500 doctors and healthcare workers have undergone specialised training under the mission. Regular online medical education sessions, case-based discussions and expert consultations have been introduced to improve treatment quality. Simultaneously, ASHA workers and Community Health Officers are conducting awareness campaigns and identifying patients at the village level. One of the programme's most significant features is the integration of advanced digital technologies. Rathore said Rajasthan has become the first state in India to develop the "MadhuRoop AI Digital Twin", an artificial intelligence platform that analyses patient health data to predict the risk of hypoglycaemia and hyperglycaemia, support clinical decision-making and enable timely medical intervention. The mission has also introduced Madhu Netra AI, an AI-powered diabetic retinopathy screening system. Rajasthan claims to be the first state in the country to implement AI-based diabetic eye screening within the public health system. The technology is capable of detecting serious retinal complications with over 90% accuracy and has already been successfully implemented in seven districts. The innovation was honoured with the chief minister Excellence Award-2026. Officials said Mission Madhuhari focuses not only on treatment but also on long-term disease management and quality of life. The programme has established a Madhuhari Bank to ensure uninterrupted availability of insulin and other essential supplies.
