India, Aug. 8 -- There is an uncomfortable pattern in many fraud investigations. The user authenticated correctly, the account had legitimate access, and the transaction moved through an approved system. Even the logs may show a fairly ordinary sequence of events. On paper, nothing appears to have broken, yet the fraud still happened.
