Los Angeles, Aug. 29 -- Just days after the death of Shelley Fabares, best known for her roles in The Donna Reed Show and Coach, her family has now revealed the cause of the actress's death.

Fabares' family confirmed the cause of her death to PEOPLE and other outlets, saying that the actress died of pneumonia after a brief illness.

The actress had earlier been diagnosed with autoimmune hepatitis and underwent a liver transplant in October 2000 after being on the waiting list for nearly two years.

In a statement announcing her death, Fabares' family remembered her for the love and warmth she shared with those around her.
