Brief Summary: This study evaluates whether an interdisciplinary case management approach based on the International Classification of Functioning, Disability and Health (ICF) and the Rehab-Cycle(R) framework can improve rehabilitation outcomes for adults with intellectual disabilities receiving community-based rehabilitation services.
