Brief Summary: The aims of this interventional study are of two-fold. First, the study aims to develop the capacity to facilitate the collaborating NGOs to address the social determinants of aging well for 3-N seniors. The second aim of the study is to evaluate the impact of the transitional support for 3-N elders. The main question is aims to answer is:
