An inquiry committee on the escape of 225 prisoners from Malir Jail in Karachi held jail authorities responsible for the incident.
According to the Inquiry Committee report, in the incident, negligence, mismanagement, and poor security arrangements were revealed.
The report said the situation worsened due to the lack of preparation and implementation of SOPs after the earthquake.
The committee recommended action against Superintendent Arshad Hussain Shah and other officers.
The report also held the Inspector General and Deputy IG Prisons responsible for the incident.
The committee further recommended that modern CCTV systems should be installed in jails.
